If you would like your donation to be in memory of a family member or friend, or in honor of someone special, LINC will send a card acknowledging your thoughtful contribution.

Please be sure to include the following information with your donation:

• The name of the honoree including:
  First Name
  Middle Initial
  and Last Name
• To whom would you like to send an acknowledgment card:
  Name (First, Middle Initial, and Last)
  Address (including City, State, and Zip Code)
  Telephone
• How you like your name displayed on the acknowledgement card?

Also, please indicate how you would like your name to appear in newsletters or other donor listings LINC may produce and distribute. If you do not want your name listed on any of our donor publications materials, please let us know and we will be sure your information remains private.
